Medicare is divided into four parts. Understanding what each does — and how they fit together — is the foundation for every other decision.
Hospital Insurance
Covers inpatient hospital stays, skilled nursing facility care, hospice, and some home health care.
Premium-free for most people who paid Medicare taxes for at least 10 years.
Medical Insurance
Covers doctor visits, outpatient care, preventive services, and durable medical equipment.
Standard 2026 premium: $202.90/month. Higher incomes pay more (IRMAA).
Medicare Advantage
An alternative to Original Medicare offered by private insurers approved by CMS.
Bundles Parts A, B, and usually D into one plan with networks and often extras.
Prescription Drug Plan
Covers prescription medications. Stand-alone with Original Medicare or built into most Medicare Advantage plans.
2026: $2,000 annual out-of-pocket cap on prescriptions.
Three Paths Forward
Once you understand the parts, you choose how to combine them. There are three common paths.
Original Medicare Alone
Original Medicare + Medigap + Part D
Medicare Advantage (Part C)
