How the callback form on this site works, what it records, and what is never asked for online. Effective September 2026.
What the callback form collects
The form asks for three things: your name, your phone number, and a question box. The question box is optional and free-form; you can leave it empty, and you should not put a Medicare number, a Social Security number, or medical details in it. Nothing else is asked for. Sent along with a submission are the page it was sent from, the time, the referring page that brought you to the site, and your browser's user agent string.
What this site never asks for
No date of birth. No health conditions, no medication list, no Medicare number, and no Social Security number. Those belong in a private conversation with a licensed agent, never in a box on a web page.
Where a submission goes
Submissions post to Zapier, the automation service that delivers them to Adam. The site itself is hosted by Vercel. Both companies handle the data on behalf of this agency and may not use it for purposes of their own.
How a submission is used
For one purpose: returning your call to talk about your insurance questions. Submissions are not sold. They are not passed to anyone else for that company's own marketing.
Phone calls
Sending the form is your agreement that a licensed agent may telephone you on the number you gave. You can ask at any point not to be contacted again, and that request will be honored.
How long submissions are kept
Contact submissions are kept for as long as they are needed to help you and to satisfy insurance record-keeping requirements, and are deleted after that.
Your choices
